Forms Manager

The Forms module of SchoolStream is to assist in the completion and submission of forms electronically that we use in our school system. Here you will be able to find a form for your particular need, complete it and submit it to the approving parties.  All parents must submit a volunteer application/background check each year in order to volunteer in any capacity at your child’s school.  It HIGHLY recommended that parents submit their volunteer application/background check at the beginning of each school year.  The process is completely done online and could take up to two weeks for the application to be processed.  You will be contacted by email upon approval.  (Please note that for security reasons our online forms are on a separate and secure website.)

Accessing Forms

To fill out and submit a form, please follow the steps below:

First, choose a form from the menu. Complete all of the required areas and choose appropriate approval name then click submit at the end of the form.  Once you have submitted the form, it will automatically be routed to the approvers to review.  After submission of form it is recommended you print your form and save your document.
